PRELIMINARY BACKGROUND APPLICATION – Police Officer
(Rev 03/2022)

Instructions to the Applicant

The information you provide in this Preliminary Background Application will be used in the background investigation to assist in determining your suitability for the position of City of Los Santos Police Officer, in accordance with San Andreas POST Commission Regulation 1953.
  • • It is your responsibility to complete this form and provide all required information.

    • This document must be filled out electronically and printed.

    • You must respond to all items and questions. If a question does not apply to you, write “DNA” (does not apply) in the space provided.

    • Send the completed form to your background investigator or the department. Do NOT send the form to POST.

(( Your application must be titled "Firstname Lastname. ))

(( Your answers should only replace the fields that say "Answer." It should not extend outside the color code box. ))

(( Do not edit the application format in any way. You may add additional fields to any area or questions or replace hyperlinks with DNA. ))

(( You may be non-select if your application is put on "Pending" status more than twice for formatting issues or missing questions. This is also the case in character. ))

(( Upon completion of this application, your character will undergo a 'psychological evaluation'. This is entirely an NPC process. It is your responsibility to inform the Personnel and Training Division if your psychological evaluation fails or shows any concerns. ))

Disqualification

There are very few automatic bases for rejection. Even issues of prior misconduct, such as prior illegal drug use, driving under the influence, theft, or even arrest or conviction are usually not, in and of themselves, automatically disqualifying. However, deliberate misstatements or omissions can and often will result in your application being rejected, regardless of the nature or reason for the misstatements/omissions. In fact, the number one reason individuals “fail” background investigations is because they deliberately withhold or misrepresent job-relevant information from their prospective employer.

  • Spoiler: show
Josh Hopkins was born at All Saints General Hospital in Los Santos on the October 6, 2000. His father was an officer in U.S Navy while his mother was a kindergarten teacher. Josh mostly passed his childhood in the West Side of Los Santos until his teenage years, after that he gradually had been in all over Los Santos, doing side hustles to make money, to see new places and all that. He was a curious, excited, and not to mention a hardworking kid. He also had a big passion for basketball. Josh selected to the school basketball team for the first time in his mid high school years. He was a decent player but his efforts could only made him and his team win the second row in the State High School Basketball Tournament for two times, other than they couldn't achieve bigger things. Because you know, in life things won't always go as how the people would wanted them to go, Josh was mature enough to know that, after all, he was raised in that kind of an environment. After finishing the high school Josh needed to leave Los Santos, he had to travel, see new places, so he decided to go to university. While his family would happily pay for his tuition, Josh didn't want them and he never asked them. Instead, he applied for scholarships on several universities relying on his athletic background. He applied for basketball scholarships to UCLA, UNLV, WSU, and lastly NYU. After the hard works of several months, New York University accepted Josh's application and granted him %75 scholarship. Josh gladly accepted it and moved to the New York City to study Tourism Management, which he believed at that time that he could have a good future if he graduate that major. While studying at the university he also worked part-time as a bartender in a pub near his apartment. By doing so, he could pay off the remaining 25 percent of his tuition fees and he could spend a stress-free educational journey. It is also worth mentioning the not only emotional but also financial support of his parents. Josh's parents was always supportive, and he has never been a freeloader or an ungrateful son to them. After finishing university Josh spent couple more months in New York, but he felt like it was the time for him to come back to San Andreas. He didn't want to be an unemployed burden for anyone so before going back to San Andreas he applied for some jobs. Finally, he received a job offer from San Fierro Tourism Agency to work as a Tourist Guide. Agency's main office was in San Fierro but they were carrying out touristic activities all around San Andreas. It was the best thing for Josh, until the bankruptcy of the company. The agency filed bankruptcy because of a managerial changes that was made by the profligate heir after the company owner's sudden death. After that Josh became unemployed. However, thanks to his smart savings he had the money to get along for quite of a time; but not doing anything, being lazy wasn't meant for Josh. So, he started looking for new jobs. At that very moment an idea hit him like a flash of inspiration; he thought of his father, maybe he should've been working for his country but it wasn't the military because he didn't want to go faraway places, he had lost that interest. He wanted to stick around, he wanted to serve his country, to touch people's lives and to witness those lives that he touched. So, he applied to Los Santos Police Department to become a police officer.
