Los Santos Police Department
PRELIMINARY BACKGROUND APPLICATION – Police Officer
(Rev 03/2022)

Instructions to the Applicant

The information you provide in this Preliminary Background Application will be used in the background investigation to assist in determining your suitability for the position of City of Los Santos Police Officer, in accordance with San Andreas POST Commission Regulation 1953.
  • • It is your responsibility to complete this form and provide all required information.

    • This document must be filled out electronically and printed.

    • You must respond to all items and questions. If a question does not apply to you, write “DNA” (does not apply) in the space provided.

    • Send the completed form to your background investigator or the department. Do NOT send the form to POST.

(( Your application must be titled "Firstname Lastname. ))

(( Your answers should only replace the fields that say "Answer." It should not extend outside the color code box. ))

(( Do not edit the application format in any way. You may add additional fields to any area or questions or replace hyperlinks with DNA. ))

(( You may be non-select if your application is put on "Pending" status more than twice for formatting issues or missing questions. This is also the case in character. ))

(( Upon completion of this application, your character will undergo a 'psychological evaluation'. This is entirely an NPC process. It is your responsibility to inform the Personnel and Training Division if your psychological evaluation fails or shows any concerns. ))

Disqualification

There are very few automatic bases for rejection. Even issues of prior misconduct, such as prior illegal drug use, driving under the influence, theft, or even arrest or conviction are usually not, in and of themselves, automatically disqualifying. However, deliberate misstatements or omissions can and often will result in your application being rejected, regardless of the nature or reason for the misstatements/omissions. In fact, the number one reason individuals “fail” background investigations is because they deliberately withhold or misrepresent job-relevant information from their prospective employer.

BOTTOM LINE: You are responsible for providing complete, accurate, and truthful responses.

          • Spoiler: show
            Ethan Cruz.
Ethan Cruz grew up in Chicago, Illinois in Cook County. He’s a 30-year-old Hispanic guy with an easygoing personality. Growing up in a big city like Chicago, he got used to a busy life full of people and noise, but he didn’t mind. His family always taught him to work hard and be kind, values he still carries with him.

In 2012, Ethan decided to try something new and moved to Las Venturas, Nevada to go to college. He enrolled at the University of Nevada, Las Venturas in August 2012 and graduated in May 2016 with a Bachelor of Arts (B.A.) in Communication Studies. He chose this major because it felt practical and gave him good people skills, but mostly, he just wanted something he could complete without too much stress.

After graduating, Ethan’s first job was at QuickMart Grocery Store as a Store Associate from June 2016 to June 2021. The job was straightforward, and he enjoyed meeting people and helping them find what they needed. His managers liked that he was reliable and always had a friendly attitude. Customers appreciated his easygoing nature, and he quickly became popular with regulars and coworkers alike. Ethan has a natural ability to listen and keep his cool, even when things get busy or stressful at work, which helps him get along well with people from all walks of life.

In July 2021, Ethan started working at a Local Auto Repair Shop as a Service Assistant until April 2023. Here, he helped out with customer service, scheduling appointments, and keeping the shop organized. His genuine, friendly attitude put customers at ease, even when they were frustrated or confused about their car troubles. His calm approach meant that people felt comfortable asking him questions, and he’d make sure they got the help they needed.
Sometimes, though, Ethan’s laid-back nature makes him seem less driven. He’s not the type to chase promotions or big career goals, which means he’s stayed in fairly simple roles despite his years of experience. He’s also not the most organized, often waiting until the last minute to complete tasks, which adds a bit of stress to his day. Some of his friends and coworkers wish he’d put a bit more energy into planning ahead or setting bigger goals, but Ethan isn’t too bothered. He’s happy to do his job, help people out, and keep things relaxed wherever he goes.
