Los Santos Police Department
PRELIMINARY BACKGROUND APPLICATION – Police Officer
(Rev 03/2022)

Instructions to the Applicant

The information you provide in this Preliminary Background Application will be used in the background investigation to assist in determining your suitability for the position of City of Los Santos Police Officer, in accordance with San Andreas POST Commission Regulation 1953.
  • • It is your responsibility to complete this form and provide all required information.

    • This document must be filled out electronically and printed.

    • You must respond to all items and questions. If a question does not apply to you, write “DNA” (does not apply) in the space provided.

    • Send the completed form to your background investigator or the department. Do NOT send the form to POST.

(( Your application must be titled "Firstname Lastname. ))

(( Your answers should only replace the fields that say "Answer." It should not extend outside the color code box. ))

(( Do not edit the application format in any way. You may add additional fields to any area or questions or replace hyperlinks with DNA. ))

(( You may be non-select if your application is put on "Pending" status more than twice for formatting issues or missing questions. This is also the case in character. ))

(( Upon completion of this application, your character will undergo a 'psychological evaluation'. This is entirely an NPC process. It is your responsibility to inform the Recruitment and Employment Division if your psychological evaluation fails or shows any concerns. ))

Disqualification

There are very few automatic bases for rejection. Even issues of prior misconduct, such as prior illegal drug use, driving under the influence, theft, or even arrest or conviction are usually not, in and of themselves, automatically disqualifying. However, deliberate misstatements or omissions can and often will result in your application being rejected, regardless of the nature or reason for the misstatements/omissions. In fact, the number one reason individuals “fail” background investigations is because they deliberately withhold or misrepresent job-relevant information from their prospective employer.

BOTTOM LINE: You are responsible for providing complete, accurate, and truthful responses.

  • Spoiler: show
Matthew was born February 1, 1994, in the US, state of San Andreas, among side the streets of Vespucci. Mother Vanessa died in childbirth, father Tyson was left alone to raise his son. His father was a marketing manager for an insurance company, therefor he had a competitive salary and performance based incensitives, that made Matthew have a good childhood and be a spoiled kid. Matthew's development took place like all normal children, having reached the right age, Matthew was sent to kindergarten, where he met other children and learned new things every day. Having reached the age of 6, Matthew went to the 1st grade of the city school. The studies were stable until the transition to high school, Matthew studied with excellent marks. In the 3rd grade, Matthew began to go to Judo training, this decision helped him a lot in the future. Moving on to high school, Matthew faced a lot of difficulties, ranging from increasing the complexity of his studies, ending with his first sympathy for girls. In honor of entering the school, Taimon gave his son a laptop, which Matthew spent all his free time on, in connection with this, Matthew had vision problems, after that Matthew was taken to an ophthalmologist who discovered that Matthew had myopia, Matthew was prescribed glasses and he continued his studies. Matthew's grades dropped to 3-4 due to the change in the school curriculum, because of this, he often felt sad and thought about how he could improve his grades in the future. A bad mood and frequent stress led him to bad company, which sent him on the wrong path, which eventually ended in a trial of a light drug, due to which Matthew was in the hospital for a week. The case was hushed up, Matthew got a lot from his parents, because of which he could not sit for several days, Matthew himself did not like to remember that incident. After a week of rehabilitation, Matthew returned to school and began to catch up with the program, while correcting his grades. The school year went faster and faster with each class. Matthew moved to the 7th grade, where for the first time he showed sympathy for his classmate, unfortunately, as it happens with children, the sympathy was non-reciprocal, which is why Matthew was also periodically sad. He tried to please girls, but girls showed zero attention to him. He continued to study for a good score, while doing Judo at the same time and taking pride of place in competitions. Matthew narrowed his circle of friends close to him every year, as he grew and understood more and more that in this world you will have a few true, loyal and respectful friends. The end of the ninth grade, the time for exams, half a year before the next annual exams, Matthew gave up Judo and began to study maths and physics, which he suffered from, it was these 2 subjects that he hardly pulled out on fours. After passing his exams, he and his two friends moved on to high school. During this period of study, he began dating a girl from a parallel class, which affected his studies. Matthew began to score on his studies, which later affected the grades, which he pulled with great difficulty to fours. Matthew will remember the graduation class for a long time, when it is necessary to decide on further admission, the time of important exams and parting with classmates. Matthew's father, Tyson, as we stated earlier, was working for an insurance company, Tyson always wanted his kid to follow his footsteps, but Matthew had another plan brewing in his mind. After writing the final exams, He decided to move out of his father's house due to his dissatisfaction with his lifestyle. Over time, he always sought to escape its restrictions, growing bored and feeling the lack of a successful career, and even a compelling story to tell his grandchildren. Pursuing a specific career path that doesn't allign with parental expectations was the main motivation that made him move away to follow his dreams. Good scores in the final exams helped him to have different ways, and choices where he can starts his career.
Matthew moved into a little appartment on Innocence Boulevard after a few months. He currently resides there and has developed connections with the locals. He accepts additional jobs to pay his bills and costs while working a part-time lawnmower job for a business close to his residence.
Growing up, Matthew was just like every normal kid, despite facing a lot of challenges, he emerged as a resilient individual. His determination helped him to overcome the struggles and obstacles he faced. Besides that, Matthew's dream was always to become a police officer, so he pursued a college education to be the strongest candidate possible. At the same time, he saw it as a way to earn a decent income in case his path to policing didn't pan out as expected.

With careful consideration of his academic achievements, personal growth, and a strong sense of community, Matthew embarked on the journey of applying for the LSPD's recruitment process. His resolute dedication to upholding justice and fostering a positive societal impact seamlessly resonated with the department's mission. Matthew's aspiration was crystalline: to bid farewell to his present life and embark on a gratifying vocation as a peace officer, wholly devoted to the advancement of justice and the enhancement of citywide safety.
